New Patient Information for Comprehensive Dentistry

You have a choice when it comes to cosmetic and restorative dentistry in Charlottesville. We’re pleased that you’ve chosen Dr. Lippard as your oral healthcare provider and are looking forward to meeting you! Your First Visit

When you visit our Charlottesville boutique dental practice for the first time, you’ll receive a comprehensive exam and a set of X-rays. Dr. Lippard will discuss your medical history, ask about medications you are currently taking, and then discuss your concerns and goals for oral health.

Your dental team will provide a tooth-by-tooth inspection, evaluate your periodontal health, assess the function of your bite, screen for oral cancer, and perform a professional cleaning. Once your thorough examination is complete, Dr. Lippard will provide you with a personalized treatment plan for your optimal oral health which may also include cosmetic dentistry suggestions to help enhance the look of your smile.

You'll find three new patient forms below. Please read and fill out the required information on each form below and return them to us via e-mail at hallippard@gmail.com with the subject header "New Patient Registration," or fax them to (434) 293-2105. You're also welcome to print them and bring them with you to your first appointment. Faxing or e-mailing the filled out forms will speed up the registration process.

Payment Methods & Dental Insurance

Your dental health and well-being is our number one concern! Dr. Lippard’s office strives to make the financial end of our relationship as simple and painless as possible by providing multiple options for payment:

  • Insurance: All insurances are accepted and filed electronically for your convenience. In addition, Dr. Lippard is a preferred provider for:   Principal/Ameritas, Anthem Blue Cross/Blue Shield, Cigna, Delta Dental, United Concordia, MetLife, and Guardian.
  • Point of Service/Balance Billing Payments: Cash, check, all major credit cards (MasterCard, Visa, Discover, American Express) and Care Credit are all accepted.
  • Custom: Other scheduled payment options may be prearranged and customized to meet your needs during treatment planning with Dr. Lippard and his staff.
